Set up emergency access on your phone

Your phone can display your Directive information even from the lock screen — so a first responder can see your decisions without unlocking your device.

iPhone

  1. Open the Health app
  2. Tap your profile picture, then Medical ID
  3. Tap Edit, then enable Show When Locked
  4. Under Medical Notes, paste the summary text from the phone setup page
  5. Add your Healthcare Agent as an Emergency Contact

Android

  1. Open Settings > Safety & Emergency
  2. Tap Emergency Information
  3. Under Medical Information, paste the summary text
  4. Add your Healthcare Agent as an Emergency Contact

Create a custom lock screen wallpaper

bloodless.org includes a wallpaper editor that places a medical emblem, your personal QR code, and alert text directly on your lock screen — so responders see it the instant they pick up your phone.

  1. Go to Lock Screen from your dashboard or choose a template from the gallery.
  2. Choose a design — pick from the template gallery (Minimal, Bold, Dark, Light) or start with a custom photo background.
  3. Drag and position — move and resize the emblem, QR code, alert text, and logo by dragging them on the preview.
  4. Customize the alert text — change "MEDICAL DIRECTIVE ON FILE" to something personal like "DIRECTIVE IN WALLET" using the text field in the Elements panel.
  5. Adjust background effects (Plus) — dim and blur your photo background so the medical elements stand out. Choose a text style (dark backdrop, light backdrop, shadow) for readability.
  6. Download — tap the download button to save the wallpaper as a PNG, then set it as your lock screen in your phone's settings.
  7. Save to account (Plus) — your design is synced to the cloud and available from any device.

Tip: Position the QR code and alert text in the lower half of the screen, below the clock area, where they won't be covered by notifications.

What to include

The phone setup page in the app generates a copyable text summary of your Directive, formatted for lock screen display. It includes your name, critical refusals, agent contact info, and a link to your full Directive.
