Click the download button to get your Directive as a PDF. Print both the full document and wallet card. The wallet card is sized to fit in your wallet or attach to your phone case.
Paper forms and the pocket card
The card view of the preview page also helps with paper: a form-fill guide gives you copy-ready blocks (name, instructions, agent, alternate) to transfer onto any printed health-care form you already own, and the Print foldable card button generates a single-sided letter sheet that folds into a signable pocket Directive — no cutting required. Language
Use the language selector to preview your Directive in different languages. Bilingual printing is useful if you travel or if your providers speak a different language.
