Steps
Choose a starting point
Visit /create and pick a template that matches your situation, or start from scratch. Templates pre-fill your blood choices but everything is editable.

Enter your name, date of birth, address, and contact details in the Principal section. This identifies you on the legal document.
Make your blood choices
For each of 10 blood products and procedures, choose Accept, Refuse, or Personal Decision. Use the "Learn more" panels to understand each item.

Designate a primary agent and an alternate who can make blood-related decisions if you're unable to communicate.

Review your complete Directive — both the full legal document and the wallet card. Download as PDF, print, sign, and witness.
